6 month reviews

It is recommended that all patients receive a comprehensive review 6 months after their stroke. In Greater Manchester, these assessments are undertaken either by the local community neuro-rehabilitation team or via the Stroke Association who have been commissioned to carry them out.

All providers of 6 month reviews in our region use the Greater Manchester Stroke Assessment Tool, which was updated and re-launched in December 2018.

Our teams are also currently collecting additional data on reviews to help us understand the unmet needs of patients. We also recently undertook training with our teams and you can download additional resources by registering on our website and accessing here.

NHS England have introduced a CQUIN to encourage implementation of reviews across the country. You can find out more here.
